Application

This agreement will enable the Lion’s Roar International office to better know you and your background in ministry association and will be kept as confidential in our files.  If you have any questions, please feel free to contact this office at your convenience.   AGREEMENT: I acknowledge and affirm that the information provided by me in this application, including all attachments and exhibits, are true and correct to the best of my knowledge.  I understand that if I am granted general membership or receive credentials of any kind through Lion’s Roar International, my participation may be withdrawn or terminated at any time and without notice due to false or misleading information. Terminations may occur due to violations of the following:

1. Living an immoral and sinful life, refusing to conform to Scriptural conduct. (I Cor. 5:11-13; Eph. 5:3-7, II Thess. 3:6-10; Gal. 5:19-21)
2. Promulgation of erroneous inconsistent and false doctrine. (Rom. 16:17, I Tim. 6:4,5; II Tim. 1:13)
3. Failure and neglect to pay renewal fees or other financial obligations. (Mal. 3:10, I Cor. 9:6-9)
4. Failure to provide 501(c) 3 information annually.

Any member who shall be under charges (in writing) shall be temporarily suspended from membership pending prompt investigation and decision.  Any member who has been disciplined has the right to appeal the action through the executive board. I hereby authorize LION’S ROAR  INTERNATIONAL, INC.  and its agents to conduct a complete investigation of my background, character, and reputation before granting me general membership in LION’S ROAR INTERNATIONAL, INC.  prior to credentialing, licensing, ordination, affiliation or association. I authorize full disclosure of information to LION’S ROAR INTERNATIONAL, INC., from past and present employers, educational institutions I have attended, all religious institutions, governmental institutions (including criminal records check), as well as any other organizations, persons, or entities with whom I’ve been associated.  Such information includes but shall not be limited to, personnel files and records, grades and grade point averages and transcripts, earned degrees, professional licenses, attendance and discipline records, complaints, suspensions, license revocations or fines and such other oral and written information which, in the exclusive discretion of LION’S ROAR INTERNATIONAL, INC. and may be relevant to determining my suitability for the general participation of any kind. I hereby release any such employer, educational or religious institution, government entity, and other persons or entity’s, and their representatives, and agents from any claim or liability of any kind for complying with such information requests from LION’S ROAR INTERNATIONAL, INC.  I also hereby release LION’S ROAR INTERNATIONAL, INC.  and its representatives and agents from any claim or liability of any kind in conducting such investigations. I authorize the making and retention of photocopies or facsimiles of all such information and request that photocopies or facsimile copies be accepted on the same basis as original documents.
